The contact form asks for your name, email address, optional telephone number, referral type, service interest and message. This information is used to respond to your inquiry, discuss requested services and coordinate appropriate next steps.

Newsletter information

If you join the newsletter, the website records your email address, optional name and subscription date. This information is used only for IReAdapt Vision information and service updates. It is not sold. You may remove your address at any time through the newsletter page or by contacting IReAdapt.

What not to submit

Do not submit Social Security numbers, financial account information, medical records, student records or other sensitive personal information through the general website forms. We can arrange a more appropriate method if protected information is required later.

How information is handled

Contact-form information is delivered to IReAdapt by email and retained only as reasonably necessary to respond, document services or meet legal and business obligations. Newsletter information is stored in a protected website data file and may also generate an administrative email notice.

Third-party services

The website may rely on the hosting provider and email provider to process and deliver inquiries. Their handling of technical information is governed by their respective privacy and security practices.
